This is splenic laceration - there is subcapsular hematoma with a splenic laceration which extends from the capsule to the hilum with an intraparenchymal hematoma

Yes, this is a splenic laceration ('rupture'); a hematoma (blue arrow) and extravasation (red arrow) are visible.
It's practically always a result of trauma. Always consider this diagnosis in a patient with low blood pressure and left upper abdominal pain.
