Initial diagnosis and management of the OPC and ER cases in all branches (neurology , cardiology , gastroenterology , respiratory system , endocrinology , nephrology , hepatology , the infectious diseases and malnutrition , immunology , hematology , oncology, follow-up of growth , development and vaccinations),admission or referral of cases requiring further management.
Neonatology skills
I can manage the following cases:
NICU cases ;
· Resuscitation in the delivery room .
· Transport to the neonatal care unit .
· Oxygen therapy through mechanical ventilation, administration of surfactant therapy (survanta), CPAP, nasal cannula ,head box ,free flow oxygen.
· Blood gases sampling and interpretation . insertion of laryngeal tubes ,insertion of intercostal tubes for the cases of pneumothorax.
· Correction of the acid base and electrolyte imbalance .
· Calculation of the amount and type of iv fluids, TPN, and feeding.
· Line insertion including peripheral lines and umbilical catheterization .
· Calculation of the doses and rate of administration of various drugs used in NICU such as prostaglandins , inotropes , respiratory stimulants , antibiotics .
· Suspicion of cases of congenital heart disease and interpretation of the hyperoxia test .
· Suspicion of cases of intraventrecular hemorrhage
· Suspicion of the cases of inborn errors of metabolism and taking samples for the metabolic screening .
· Suspicion of cases of sepsis ,NEC , and their stabilization and NICU management.
Intermediate care cases:
· Management of cases of neonatal jaundice through phototherapy ,IVIG, (with possible transfer to NICU for exchange transfusion) ,TTN. infant of diabetic mothers for follow-up and management of complications such as hypoglycemia , respiratory distress, CHD
Normal care cases:
· Including resuscitation in the delivery room ,neonatal transport, determination of the gestational age ,and the growth parameters, management of infant of HBS antigen positive mother .
PICU SKILLS
Including the mechanical ventilation skills ,peritoneal dialysis, exchange transfusions, cvp measurement, central and peripheral lines insertion, insertion of ET tubes ,dealing with all cases of cardiorespiratory emergencies and failure, dealing with all cases of endocrine emergencies including DKA, dealing with sepsis and DIC, renal failure, hepatic failure and encephalopathy, CNS infections, electrolyte and acid bases and blood gases disturbances, arrhythmias including medical treatment and DC shock, basic and advanced life support maneuvers.
