He needs to visit his doctor to arrange investigations to find it and to arrange a surgery for removal if needed. Ability to get children depends on many factors, semen analysis may be required, but fortunately many men were able to get children with just 1 testis, so no need to worry.

Thank you for your advise. He feel heat in and near by portion, recently I had a talk with him to know about the problem he is facing. And the fluid is just like water not normal as it should be.
