    The diagnosis is Athererosclerosis. The picture is of an earlobe crease which is highly suggestive of this diagnosis.

    Frank`s sign. A diagonal crease in each earlobe is referred to as Frank’s sign. This sign was originally described as a marker of coronary artery disease, with a moderate sensitivity (approximately 48%) and specificity (approximately 88%). Frank’s sign has been subsequently associated with other cardiovascular risk factors.
