How To Practice Medicine In Poland For Foreign Doctors

    graduated from a Polish medical university and never employed after the graduation;

    Documents required by ORL may differ in regards to any particular category of the foreign nationals applying for the right to conduct professional activities. They have been listed below.

    Notice:

    In Poland any person conducting activities belonging to the profession of a physician or dentist without an appropriate licence granted by Chamber of Physicians and Dentists violates the legal regulations currently in force and is subject to criminal penalties on the basis of art. 58 item 1 of the Act on the Profession of a Physician of December 05th, 1996.

    I.

    Information for physicians and dentists of foreign nationality - graduates of Polish medical universities applying for the limited licence to practise their profession in order to complete internship.


    A foreign national who holds a diploma of physician (stomatologist / dentist) issued by a Polish medical university and intends to complete internship in the territory of RP shall submit the following documents to an appropriate ORL (partially the following are the same as the ones required in order to obtain the Motion of the Minister of Health and Social Welfare):

    1. Motion of the Minister of Health and Social Welfare for granting the limited licence to conduct professional activities;

    2. Application of the physician for granting the limited licence to practise the profession of a physician / stomatologist, membership of and entering into the register of regional chamber of physicians and dentists (form to be obtained from OIL);

    3. copy of a passport and visa or a permission to stay in RP;

    4. copy of the diploma of physician (stomatologist);

    5. certificate of health issued by a II degree specialist of internal diseases or specialist of occupational medicine being a member of the chamber within which territory the physician intends to complete his internship;

    6. statement of capacity to perform acts in law;

    7. acceptance for the internship issued by the training institution;

    8. financing conditions of the internship.
    If the study at a Polish medical university was completed in a language other than Polish, the physician is required to submit a certificate of state examination in the Polish language.

    II.

    Information for physicians and dentists of foreign nationality - graduates of Polish medical universities who practised the profession abroad and for the first time apply for the limited licence to practise the profession of a physician in RP

    A foreign national who holds a diploma of physician (stomatologist / dentist) issued by a Polish medical university, practised his profession abroad and for the first time applies for the licence to practise the profession of a physician or stomatologist in Poland shall submit the following documents to an appropriate ORL (partially the following are the same as the ones required in order to obtain the Motion of the Minister of Health and Social Welfare):

    1. Motion of the Minister of Health and Social Welfare for granting the limited licence to conduct professional activities;

    2. Application of the physician for the limited licence to practise the profession of a physician / stomatologist, membership of and entering into the register of regional chamber of physicians and dentists (form to be obtained from OIL);

    3. copy of a passport and visa or a permission to stay in RP;

    4. copy of the diploma of physician (stomatologist);

    5. certificate of health issued by a II degree specialist of internal diseases or specialist of occupational medicine being a member of the chamber within which territory the foreign national intends to conduct his professional activities;

    6. statement of capacity to perform acts in law;

    7. evidence of authorisation to conduct professional activities in the country where physician previously practised his profession;

    8. evidence certifying a completed internship in the country where the physician for the first time obtained the licence to conduct professional activities;

    9. an extract from judiciary record issued by the country where the physician previously practised his profession;

    10. proof of professional conduct issued by the medical chamber where the foreign national previously conducted his professional activities;

    11. certificate of employment, including an opinion of professional competence;
    if the physician of foreign nationality intends to complete a post-graduate training course in RP:

    • acceptance of the training course issued by the training institution;

    • financing conditions of the post-graduate / preparatory training course.
    if after completion of the post-graduate training course, the physician of foreign nationality intends to practise his profession in order to gain profit:

    • promised employment from a health care institution, allowing completion of internship at the initial stage;

    • opinion of a regional self-government of physicians or an appropriate delegate to a Regional Conference of Physicians;

    • addresses of two individuals or institutions willing to provide references upon the request of ORL.
    If the study at a Polish medical university was completed in a language other than Polish, the physician of foreign nationality is required to submit a certificate of state examination in the Polish language organised by NRL.

    Documents submitted to ORL shall be accompanied by a current address for correspondence and, if possible, by a telephone number.

    If the foreign national completed an internship abroad that is compatible with requirements applicable in the case of internship in Poland, after obtaining the limited licence to conduct professional activities he can apply to the Minister of Health and Social Welfare for recognition of his internship completed abroad, in whole or partially. If the internship completed abroad is recognised as equivalent to the internship required in Poland, ORL upon the motion put forward by an appropriate committee for education shall set out the period of an adaptation training course that the physician shall be required to complete before taking the state examination. The examination shall be introduced on January 01st, 1999, at present the physician can apply for the right to practise his profession immediately after completing the adaptation training course.

    III.

    Information for physicians and dentists of foreign nationality - graduates of foreign medical universities who have never practised the profession and apply for the limited licence to practise the profession in RP

    A foreign national who holds a diploma of physician (stomatologist / dentist) issued by a foreign medical university, has never practised his profession abroad and for the first time applies for the licence to practise the profession of a physician or stomatologist shall submit the following documents to an appropriate ORL (partially the following are the same as the ones required in order to obtain the Motion of the Minister of Health and Social Welfare):

    1. Motion of the Minister of Health and Social Welfare for granting the limited right to conduct professional activities;

    2. Application of the physician for the limited licence to practise the profession of a physician / stomatologist, membership of and entering into the register of regional chamber of physicians and dentists (form to be obtained from OIL);

    3. copy of a passport and visa or a permission to stay in RP;

    4. translation into the Polish language and a copy of the diploma of physician (stomatologist);

    5. certificate of equivalency of the above diploma issued by MZiOS or a nostrification of a foreign diploma issued by any Polish medical university;

    6. certificate of health issued by a II degree specialist of internal diseases or specialist of occupational medicine being a member of the chamber within which territory the foreign national intends to conduct his professional activities;

    7. statement of capacity to perform acts in law;

    8. certificate of state examination in the Polish language organised by NRL;

    9. certificate of examination in the field of the medical ethics code and legal regulations concerning the profession of physician, organised by NRL (till the introduction of a state examination completing the post-graduate training course);

    10. extract from judiciary record issued by the country the physician comes from;
    if the physician of foreign nationality intends to complete a post-graduate training course in RP:

    • acceptance for the training course issued by the training institution, including provisions allowing the internship to be completed during the initial stage of the course;

    • financing conditions of the post-graduate training course.
    if the physician of foreign nationality intends to practise his profession in order to gain profit:

    • promised employment from a health care institution, allowing completion of internship at the initial stage;

    • opinion of a regional self-government of physicians.
    Documents submitted to ORL shall be accompanied by a current address for correspondence and, if possible, by a telephone number.

    V.

    Information for physicians and dentists of foreign nationality who have been granted the limited licence to practise the profession of a physician in the territory of RP, completed the post-graduate / preparatory training course or adaptation course, taken the state examination and apply for the licence to practise the profession of a physician in RP.

    A physician of foreign nationality who has completed the post-graduate training course or adaptation course, taken the state examination and intends to practise the profession of a physician or stomatologist in the territory of RP shall submit the following documents to an appropriate ORL (partially the following are the same as the ones required in order to obtain the Motion of the Minister of Health and Social Welfare):

    1. Motion of the Minister of Health and Social Welfare for granting the right to conduct professional activities;

    2. Application of the physician for granting the licence to practise the profession of a physician / stomatologist, membership of and entering into the register of regional chamber of physicians and dentists (form to be obtained from OIL);

    3. Limited licence to practise the profession of a physician, limited licence to practise the profession of a stomatologist or certificate of the licence to practise the profession of a stomatologist under the supervision of a stomatologist authorised to practise his profession independently;

    4. Record of the internship or certificate of adaptation course in the field of general medicine and dentistry;

    5. Certificate of a state examination (after January 01st, 1999);

    6. Current certificate of health issued by a II degree specialist of internal diseases or specialist of occupational medicine being a member of the chamber within which territory the foreign national intends to conduct his professional activities;

    7. statement of capacity to perform acts in law;
    if the physician of foreign nationality intends to complete a post-graduate training course in RP:

    • acceptance of the training course issued by the training institution,

    • financing conditions of the training course.
    if the foreign national intends to practise his profession as an employed physician in order to gain profit:

    • promised employment as a physician from a health care institution;

    • opinion of a regional self-government of physicians.
    Opinion concerning his professional competence issued by the health care institution in which the physician has completed the post-graduate training course.

    VI.

    Information for physicians and dentists of foreign nationality who have been granted the licence to practise the profession of a physician (MZiOS licence to practise the profession of a physician) in RP for a limited period of time and apply for renewal of the licence to practise the profession of a physician in RP.


    A physician of foreign nationality who has been granted the licence to conduct professional activities (licence of the Ministry of Health and Social Welfare to practise the profession of a physician) in the territory of RP for a limited period of time and applies for its renewal shall not later than two months before its expiry date submit the following documents to an appropriate ORL (partially the following are the same as the ones required in order to obtain the Motion of the Minister of Health and Social Welfare):

    1. Motion of the Minister of Health and Social Welfare for granting the licence to conduct professional activities;

    2. Application of the physician for granting the licence to practise the profession of a physician / stomatologist, membership of and entering into the register of regional chamber of physicians and dentists (form to be obtained from OIL);

    3. Licence to practise the profession of a physician or right to practise the profession of a stomatologist, so far being the basis for conducting his professional activities;

    4. Current certificate of health issued by a specialist of internal diseases or a specialist of occupational medicine being a member of a chamber of physicians and dentists;

    5. Statement of capacity to perform acts in law;
    If the physician of foreign nationality intends to complete a post-graduate training course in RP:

    • acceptance of the training course continuation issued by the training institution,

    • financing conditions of the training course.
    If the foreign national intends to continue to practise his profession as an employed physician in order to gain profit:

    • promised employment as a physician from a health care institution;

    • opinion of a regional self-government of physicians;

    • opinion on his professional competence issued by the health care institution he was employed by;
    If the foreign national intends to continue a private practice only – opinion on his professional competence issued by a physician authorised by ORL;

    If the foreign national intends to take up the professional activities as a self-employed physician in order to gain profit – opinion of an appropriate committee of ORL in charge of individual practice regarding meeting all and any requirements set out in art. 50 item 1 – 4 of the Act on the Profession of a Physician.
