    A 38 year old woman presents to the emergency room complaining of cough, fever and shortness of breath. She has a history of thrombotic thrombocytopenic purpura. She underwent plasma exchange with fresh frozen plasma last week. She has improved significantly after one week of treatment. Her pulse is 90/min, blood pressure 120/80 mmHg, temperature 101.2 F and O2 saturation is 80% on room air. Chest x-ray shows diffuse opacifications on both lungs.
