USMLE CS Cases: Ligament Damage

    History :
    HPI: 34 yo F c/o knee pain for 10 days which started after returning from hiking trip on rocky
    mountains. Pain is present on the lateral side of the right knee, 5/10, non radiating, dull, aggravates
    with walking/movement and partially relieved with OTC ibuprofen. She denies popping sound. She felt
    a give away feeling when the pain started. There is no
    redness/swelling/warmth/numbness/tingling/weakness. She denies fever, rash, vaginal discharge,
    direct trauma or fall. There is no other joint involvement. The pain is badly affecting her lifestyle as she
    works as a waitress and can not stand too long.
    ROS: no weight/appetite changes, no urinary/bowel changes, no nausea/vomiting.
    PMH/PSH/FH: none
    Allergies: aspirin causes a rash.
    Medication: OTC Ibuprofen.
    S/H: non smoker, non alcoholic, no recreational drug use, sexually active with spouse, waitress.
    Physical Examination :
    Patient is NAD
    vitals: copy paste
    Extremities: Right knee is normal on inspection, tenderness on palpation, painful and reduced ROM.
    Vaurus test is positive. Posterior tibial pulse is intact B/L.
    Neuro: Lowerlimb Motor : Power 5/5, DTR intact B/L. Sensory: sensations intact B/L
    Diagnosis 1 :
    Ligament damage
    Diagnosis 2 :
    Meniscus tear
    History Findings Physical Exam Findings
    1) Right knee pain for 10 days after
    hiking trip
    1) Varus test posiitve
    2) pain on lateral side of the right knee
    3) aggravates on movement
    4) Patient had a giving away feeling
    when the pain started.

    Diagnosis 3 :
    Diagnostic Study/Studies - Labs
    1) CBC with differential count
    2) Xray right knee.
    3) MRI right knee
