    Dear Dr Heba
you should take full history as I told you
analysis this symptom, describe it in details "onset, duration, course what exaggerates this symptom and what reliefs it, type:tremors or what?? "
    ask about other symptoms
you should ask about any medications she takes ?
her vital data: her pressure, may be she has postural hypotension? her pulse ?
    do neurological examinations
may be there is an electrolyte defficency ?? vitamin deficinecy??
make sure her thyroid functions is okay ?? and so on ?
you can get all this data by taking good history so we can reach a Differential diagnosis at least ...

Your description refers to Sydenham’s chorea following acute
    rheumatic fever (ARF) resulting from
childhood infection with Group A beta-
    hemolytic Streptococcus
but thorough history supported by adequate investigations should be done.
