I am in America. Medical school is a professional school, meaning that you first must obtain an undergraduate degree, also know as a bachelor's degree, which is usually 4 yrs. Then you take the Medical College Admissions Test, MCAT, and apply. Medical School is 4 yrs after that:

Medical Sciences: 2 yrs (You take USMLE Step 1 at the end of your second year)
Clinical Years: 2 ( By the end of your fourth year you must have taken both Step 2 and the Clinical Skills part as well before graduating)

so basically to obtain an MD in USA its about 8 years total. From here you match into residency of choice.

In Mexico (Sonora) it is 4.5 years of Med School (9 semesters), then 12 months of internship and then 12 months of social service. So, in total 13 semesters to be a General Physician.

Pre-medical course can usually be a biology-related course. But there are doctors whose pre-medical degrees are engineering, philosophy, accounting, etc. The requirements for medical school are usually taken in the first 2 years of pre-med, however, the CHED (Commission on Higher Education) stipulated that prior to entering Medicine, one has to finish a Bachelor of Science/Bachelor of Arts degree which takes 4 years. The only exception is the University of the Philippines, which has an INTRAMED program where pre-med subjects are taken in the first 2 years, then they go straight to medical school at UP College of Medicine. Admission to the program is by merit basis and highly competitive. I know students have to sacrifice their summer vacation for INTRAMED. The University of Santo Tomas used to have an accelerated BS Biology program, taken in 3 years (also sacrificing their summer vacations), but was closed because most students prefer the more relaxed 4 year BS Biology program. Prior to entrance to medical school, a candidate has to take the NMAT (National Medical Admissions Test), similar to the US MCAT. Students in the upper 10-15% echelon are given preference to the top medical schools in the country (UST, and UP). Medical school begins with basic sciences (anatomy, biochemistry, physiology, neuroanatomy, histology) taken in the first year. 2nd year emphasis are the pathologic subjects (pathology, microbiology, parasitology) and primer in clinical sciences (surgery 1, medicine 1, paediatrics 1, obstetrics 1, pharmacology, psychiatry 1, community medicine 1), with beginning patient interaction in the wards. 3rd year subjects are heavy on major clinical specialities (surgery 2, urology, neurology, medicine 2, paediatrics 2, obstetrics 2, gynaecology, psychiatry 2, rehabilitation medicine, community medicine 2, ophthalmology, otolaryngology). 4th year or clerkship is one year, where the students now do duties in the hospital. It used to be 24 hours duty + another 8-12 hours regular duty the next day, usually done every 3 days. Prior to graduation, you have to pass the oral revalida (oral examinations, where the clerk is subjected to basic sciences, clinical sciences, actual patient in the clinic or hospital, and discussion of medical/surgical emergency. After graduating from medical school, you have to take a 1 year general internship program (equivalent to PGY-1 in the US/Canada), which is a requirement for the medical licensure examination. After passig the licensure exam, then that is the time you apply for residency, the length of training depends on the subspecialty (3-6 years). Fellowship is another 1-4 years depending of course on the specialty.
